The clinical characteristics of renal aging

Changes in renal structure

GFR reduced

GFR decreased with age, down by about 3.8 ml / min / 1.73 m2 per year. According to studies have shown that the rate of GFR decline increases with age.

Renal tubular hypofunction

The main features of aging are progressive renal tubular dysfunction, reduced sodium reabsorption, potassium excretion, and reduced urine concentration, resulting in an increased risk of AKI. In elderly patients, the renal tubular potassium gradient decreased and the potassium excretion in the distal renal tubule decreased.

Vascular lesions

Alterations in the structure and function of the renal vasculature may occur in elderly people: increased deposition of extracellular matrix (ECM), increased neointimal proliferation in the anterior arteriole, and increased intrarenal shunt and capillary bypass.

Renal sympathetic nerve excitability, increased vasoconstriction, and aortic baroreceptor sensitivity decreased. Renal vasodilators, such as atrial natriuretic peptide, nitric oxide (NO), and amino acids, decrease in effectiveness. 

Kidney aging cell cycle progression

With the increase of age G1 / S phase cell cycle gradually stagnated, p16 positive aging cells were expressed. G2 / M cell cycle arrest has different effects on different cells, can prevent smooth muscle cell intimal hyperplasia.
